If you're a new patient and would like to proactively prepare for your initial visit, please print and fill out the forms above.
Please bring the forms with you to your appointment, so a staff member can upload it to your profile.

To receive a copy of your medical records, you'll need to authorize the medical facility holding them permission to release them. You'll need to do this with the your ink-to-paper signature.
Prescription refills should be listed on your prescription packaging. If you have refills listed on your current prescription, please contact your pharmacy for refills.
If you no longer have prescription refills for a medical condition that requires maintenance medication (e.g. for high blood pressure, diabetes, or cholesterol) and you need a prescription renewed, then you will need to schedule a follow up appointment.
